Who we are

Plan Health Check (“we,” “us”) operates this website for educational purposes related to 401(k) and defined-contribution retirement plans. You can reach us at hello@planhealthcheck.com.

Information we collect

We collect information you voluntarily provide — for example, the name, email address, company, and message you submit through our contact form. We may also collect limited technical information through standard web server logs (IP address, user agent, referring URL, pages viewed, timestamp) to maintain and secure the site.

The Plan Lookup tool on this site operates against a static dataset sourced from public DOL Form 5500 filings. The search queries you enter are processed in your browser; we do not currently log individual search queries to a server.

How we use information

We use the information you provide to respond to your inquiry, connect you (with your consent) to an unaffiliated licensed advisor for a plan review, and send follow-up communications about your request. We do not sell your personal information.

Data sharing

If you request a referral to a licensed advisor, we share the information you have provided with that advisor so they can contact you. Those advisors are independent third parties with their own privacy practices; we encourage you to review their disclosures.

We may use standard third-party service providers for website hosting (Cloudflare Pages), email delivery, and analytics. Each processes data on our behalf under their own terms.

Cookies

This site uses only strictly necessary cookies for core functionality. We do not currently use advertising cookies or cross-site tracking.

Your choices

You can request that we delete the information you have shared with us by emailing hello@planhealthcheck.com. We will comply promptly unless applicable law requires us to retain specific records.
